臨床質子治療射束劑量量測之探討

The Study of Dose Measurement in Clinical Proton Beam Therapy

摘要


質子射束在治療腫瘤的臨床應用上正以可觀的速率增加,世界各國大型醫學中心皆相繼宣布設立質子治療中心,而臺灣第一部質子治療中心為林口長庚醫院,目前已在建造中。質子治療中心設備的組成、精準度、劑量曲線、劑量學參數特性與吸收劑量的測量方法等,對相關的醫學物理人員都是新的知識。本研究參酌相關文獻美國醫學物理師學會(American Association of Physicists in Medicine, AAPM)第16號報告、國際輻射單位與度量委員會(International Commission on Radiation Units and Measurements, ICRU)第59號報告、國際原子能總署(International Atomic Energy Agency, IAEA)第398號報告,介紹質子參考劑量之測量儀器建議。諸多文獻建議以量熱劑量計、法拉第杯或游離腔量測作為質子吸收劑量的絕對測量方式。測量質子射束的劑量計方面,應由(上標 60)Coγ-ray造成的空氣吸收劑量ND或水吸收劑量N(下標 D.W.Q)來測定。未來質子治療設備完成後,在驗收測試時,應對其劑量學參數進行詳細評估,而監測單位也應定期校準。希望藉由國外的質子射束量測劑量建議書,來維護質子治療的品質,並能提供質子劑量測量及劑量校正等相關資訊供國內醫院參考。

The clinical application of proton beam in radiation oncology keeps increasing, and many medical centers in the world announced that they also want to establish proton therapy centers. The first proton therapy center in Taiwan will be located in Linkou of the Chang Gung Memorial Hospital and is under construction. The facilities, accuracies, dose curves, dose parameters characteristics and the measurement of absorbed dose etc, are new knowledge for medical physicists. The study combines AAPM TG 16 report, ICRU No. 59 report and IAEA No. 398 report introducing the suggestions for proton dose measurement. Many references suggest that we can use calorimeters, Faraday Cups or ionization chambers may be used to measure the absorbed dose, and air absorbed dose of cobalt-60 or absorbed dose of water should be applied. After the facility of proton therapy is established, detailed evaluation of dose parameters and calibration of the monitor unit, as well as keeping good quality of proton therapy by consulting foreign references, should be done regularly. We expect that this study can provide the hospitals with important information regarding proton dose measurement and calibration methods.
