C型臂錐形射束電腦斷層應用於肺部病灶作經皮肺穿刺術之研究

The Study of Percutaneous Lung Biopsy under C-arm Cone-Beam CT Guidance

摘要


本研究目的以C型臂錐形射束電腦斷層(C-arm Cone-Beam CT, CBCT)系統結合透視X光導引經皮肺穿刺術來診斷肺部病灶,並且與其他檢查方式作比較。本研究將收集2012年12月至2013年12月間,43位病患(男性25人,女性18人,平均年齡為66.9 ± 13.2歲),共計43個病灶處(平均大小28.2 ± 13.4 mm),因肺部病灶接受CBCT導引經皮肺穿刺術,並評估其輻射劑量、敏感性、特異性、準確率及併發症發生率。本研究結果顯示,病患中有38個是惡性,5個是良性,而敏感度、特異度、分別為97.4%(37/38),100%(5/5),診斷準確度97.7% (42/43)。併發症中有11位產生氣胸(25.6%),4位有發生喀血(9.3%)。以CBCT導引經皮肺穿刺術來診斷肺部病灶是一個高診斷率及安全的方法。

The purpose of this study was utilized CBCT guidance system combined with X-ray fluoroscopy to diagnose lung nodules, and compare with other methods. From Dec, 2012 to Dec, 2013, 43 patients (25 male, 18 female; mean age, 66.9 ± 13.2 years) with 43 lung nodules (mean size, 28.2 ± 13.4 mm) underwent percutaneous lung biopsy with CBCT guidance system were evaluated. Descriptions of the examination data were including radiation dose, sensitivity, specificity, diagnostic accuracy and complication rates. Thirty-eight nodules were diagnosed as malignant, five as benign. The sensitivity, specificity and diagnostic accuracy were 97.4%, 100% and 97.7%. Pneumothorax and hemoptysis were found in 11 patients (11/43, 25.6%) and 4 patients (4/43, 9.3%). CBCT guidance system is a highly accurate and safe diagnostic method to diagnose lung nodules.
