應用田口方法於CIRS型假體評估乳房攝影參數之最佳化

Optimizing the Image Quality of Mammography Using Taguchi Analysis: A CIRS Phantom Study

摘要


本文應用田口方法於CIRS 010B 型假體評估乳房攝影最佳化參數以提高其影像品質。目前針對乳房篩檢,乳房X 光攝影為主要篩檢儀器,而本實驗是利用CIRS 010B 型乳房模擬假體與田口L_8(2^7)直交表搭配,重複進行數次的實驗,以找出最佳影像品質之特性條件組合,最後再與儀器自動曝射控制(AEC)的參數比較。經由田口最佳化分析後,發現最佳化的參數組合為鈣化點模擬物的最佳化參數為:32 kVp、56 mAs、Target/Filter (Mo/Mo)、FOV(18×24)。纖維模擬物最佳化參數為:28 kVp、56 mAs、Target/Filter (Mo/Mo)、FOV(18×24)。腫塊模擬物的最佳化參數設定為:32 kVp、56 mAs、Target/Filter (Mo/Mo)、FOV(18×24)。使用變異數分析ANOVA(Analysis of Variance)驗證研究所得結果,亦顯示95%以上信心水準,具有統計上顯著的意義。結論是纖維在平均乳腺劑量(AGD)與S/N 值與腫塊的S/N 值皆優於儀器自動曝射控制,雖然鈣化點的AGD 與S/N 值與腫塊的AGD 值不如儀器自動曝射控制,但其值並未超過法規所規範的限制值,還是可以做為臨床選用。

Optimization of the mammography imaging quality according to the CIRS 010B phantom was accomplished using the Taguchi analysis. An orthogonal array L_8(2^7) that was recommended by the Taguchi was adopted as the analytical technique in this work. Every combination of the operating factors of mammography was repeated 3-5 times to ensure the reproducibility then compared to outcome from the system default automatic exposure control (AEC). Optimal results were: calcification mimetic: 32 kVp, 56 mAs, Target/Filter (Mo/Mo), FOV (18×24); Fiber mimetic: 28 kVp, 56 mAs, Target/Filter (Mo/Mo), FOV (18×24); and mass mimetic 32 kVp, 56 mAs, Target/Filter (Mo/Mo), FOV (18×24), respectively in this work. The derived were also verified by the ANOVA of 95% confidence level. The optimal suggestion was superior to the AEC in fiber for either AGD or S/N, and in mass for only S/N. The inferior result for optimal recommendation in identifying the calcification to the AEC might be the high exposed dose, although the dose was still below the recommended one by the medical physicists. The comparatively contrast imaging quality obtained from the optimal combination in mammography was still recommended from the clinical viewpoint.
