台灣中南部地區身心障礙牙科就醫狀況調查及牙科醫療服務網路建立成效評估

摘要

目的：口腔疾病就醫是一般人普遍接受的牙科就診行為，但對身心障礙的民眾來說，則是困難且重要的課題。為了強化身心障礙牙科醫療服務網絡，需將社福、教育、醫療及早療等四大方向之服務網建立及互相連結成交錯的網路，將牙科醫療網路的建立推展到每位身心障礙之病患，提供專業、適當的個案管理、就醫服務與衛生教育，以助身心障礙者之口腔保健。方法：本研究以台灣中南部地區（雲林縣、嘉義縣、嘉義市、台南縣及台南市）為基礎，運用問卷調查的方式，了解身心障礙牙科就醫狀況。而後以此調查之結果，以SWOT策略分析研擬牙科醫療服務網路建立之模式並進行成效評估。結果：身心障礙者及其主要照顧者對於牙科醫療服務的需求很高，期盼可以獲得較好的醫療環境和品質。而有意願加入身心障礙醫療院所的牙科醫師，佔有一定比例。特殊教育學校、特教班級、及教養院之教師及教保員，對潔牙衛教有高度的滿意度，及渴望本推廣計畫可以繼續全面實施。結論：本研究加強身心障礙者對於牙科醫療就醫資訊之取得、及就醫方便性之提升，並對於口腔自我照護能力能自早療階段即可獲得加強。此研究結果可提供全台灣推展身心障礙牙科醫療之參考。

Purposes: Receiving dental treatment is ordinary in general population. In disability patients, dental treatment is difficult but important issue. In order to provide good dental care to every disability patients, crossly link the networks of social welfare, education, dentistry, and early intervention of developmental delay children are needed. By providing professional case management, dental treatment, and oral hygiene instruction, the disability patient can have good oral health. Methods: The questionnaires of dental treatment in disability patients, oral hygienists, and dentists in mid-southern Taiwan were used in the study. By the result of the questionnaires and analyzed by SWOT strategy assessment, the good dentistry network for disability patient was provided and practiced. Results: The dentistry network was assessed by questionnaire. The disability patients and their primary care providers demanded for dental treatment and hoped for good environment and quality. There were many dentists who wish to provide the dental treatments of disability patients. The teachers and nurseries of special education school were highly satisfied with oral hygiene instruction and hoped there were continuous and full scale projects for disability dentistry network. Conclusion: The study helped the disability patients to get the dental treatment information and provide the assessability of dental care. The study also improved the oral care since early intervention stage on developmental delay children. The results of the study can provide the practical pattern of dentistry network in Taiwan.
