嬰幼兒期口腔健康預期指引的臨床應用與病例報告

摘要


口腔健康預期指引是早期防治的觀念,著重嬰幼兒口腔健康的管理,藉由疾病探知(disease detection)、進而預防疾病(disease prevention)、達成促進健康(health promotion)的目標。在小兒科的領域內,使用預期性指引進行家長衛教,增加父母對於孩童健康的資訊管理,已有相當的歷史。嬰兒口腔健康預期指引共有六個部分,分別是:口腔和牙齒的發育、氟化物的使用、飲食與營養、口腔衛生、口腔習癖、以及外傷和意外傷害的預防。這些指引內容經由表格化後,能夠快速提供臨床醫師諮詢家長時的備忘資訊和確認。當實際應用在臨床上,也會根據每個孩童的個別性而有不同建議指導,期望能以預防的觀點出發而降低口腔疾病的發生率。本篇文章的目的主要在推廣臨床上口腔健康預期指引的應用,以口腔健康指引內容的描述為基礎,並提供一個臨床上的病例以供諮詢參考。而實際上口腔健康預期指引的臨床效用,則還需要更多的研究討論和長期追蹤分析。

Anticipatory guidance is a concept of early intervention emphasized on infant and toddler oral health management. This tool is designed to achieve goals in disease detection, disease prevention and health promotion. Pediatricians have utilized this tool for parent education in order to enhance the health information resources for their children in years. There are six sections with regards to oral and dental development, fluoride, diet and nutrition, oral hygiene, non-nutritive habit and injury prevention. These guidelines could be as a timely reference during clinical consultation with parent with the formation of a chart. Each child’s early intervention suggestion will be based on individual needs in order to reduce the incidence of oral disease. The purpose of this article is to promote the concept of anticipatory guidance toward pediatric oral health, elaborate its content and report two cases for clinical utilization. Further longitudinal data collection and analysis for pediatric dental professional is highly expected.
