Odontoma is one of the common types of mixed odontogenic tumors. It is composed of enamel, dentin, and variable amounts of cementum and pulp tissue. A compound odontoma is composed of multiple small malformed tooth structures. This case is a 10-year-old boy in his mixed dentition, he had compound odontoma over mandibular premolar area which caused ectopic eruption and possible impaction of the underneath first permanent premolar. After early detection, radiographic (CT scan) diagnosis, and space analysis, the odontoma was removed under general anesthesia and the space maintainer was delivered. After surgery, the first premolar self-corrected the path of eruption and erupted. Early detection and removal of the odontoma is therefore recommended for preventing further serious complications.
