Chronic inflammation of endodontically treated primary tooth may gradually progress. Developmental problems of the successor may happen without timely intervention. We present a case of 9-year-6 month-old girl with apical periodontitis of mandibular left first primary molar when she first visited our OPD. The patient didn't show up for scheduled appointment. After three and half years, the patient came back to our OPD and the mandibular left first premolar was horizontally impacted. Primary molar was extracted and then comprehensive orthodontic treatment with miniscrew was performed to tract the premolar, aligned the teeth and accomplished normal occlusion.
