嚴重早期性齲齒與牙釉質發育不良之相關性:病例報告

Association Between Severe Early Childhood Caries and Enamel Development Defect: Case Series

摘要


兒童嚴重早發性齲齒影響了孩童的生活品質及健康狀況。其風險因子包含了夜奶、含糖飲食、牙釉質之缺陷或特殊照護需求。本篇兩名兒童嚴重早發性齲齒案例介於25至29個月大,其dmft指數超過17。幾乎每顆萌發的牙齒表面皆有齲齒。口內檢查顯示有牙釉質發育不良情形。而齲齒呈現牙釉質之大量缺損,及暴露的受感染牙本質,最後因行為因素,於全身麻醉下完成全口治療及重建。然而,並非所有兒童嚴重早發性齲齒的dmft指數都會超過17。根據台灣最新的調查,2-3歲孩童其平均的dmft指數為0.51。因此,可能有其他原因,例如牙釉質發育不良,造成幼兒的兒童嚴重早發性齲齒。謹慎的口內檢查及早期的介入,或許能提供孩童更好的口腔健康及生活品質。

Severe early childhood caries impact children's life quality and healthy condition. It may be attributed to different risk factors, including nocturnal feeding, sugar-containing snacks, enamel defects or special care needs. Two cases in this series from 25 to 29 months of age present severe early childhood caries. The decayed, missing and filled teeth (dmft) index over 17. Almost each erupted teeth surface were decayed. Intra-oral examination showed enamel hypoplasia. Decayed teeth appeared complete enamel loss and soften dentin. Full mouth rehabilitation were done under general anesthesia because of uncooperative behavior. However, not every severe childhood caries patient's dmft index exceed 17. According to the lates investigation in Taiwan, the dmft index among 24-35 months children is 0.51. It may exist other reason such as enamel development defect causes more severe childhood caries in younger patient. Careful intra-oral examination and early intervention may offer better oral health and life quality.
