某精神專科醫院五年職能治療階段趨勢變化及其對工作復健獎勵金影響之回溯世代研究

A Retrospective Study on the Five-Year Changes in Occupational Therapy Stage for Patients in a Psychiatric Hospital and its Impact on Work Rehabilitation Bonus

摘要


目的:(1)瞭解2017年~2021年間進階、維持階、退階、有進有退及各細項之分布情形;(2)比較五年內月職能階段、世代初始年月職能階段、五年內職能階段變化之年平均工作復健獎勵金差異情形。方法:資料來源為中部某精神專科醫院5階多元職能資料庫及工作復健獎勵金資料庫,取2017年1月~2021年12月住院病友及某社區復健中心進行工作復健的學員。研究目的(1)之樣本為42,192人月(812人);研究目的(2)之樣本為16,533人月(304人)。結果:5年期間進階率25.6%(平均花費時間1.7±1.2年),維持率11.0%(平均花費時間4.9±0.4年),退階率38.1%(平均花費時間2.1±1.3年),有進有退率25.3%。在工作復健獎勵金部分,經控制混淆因素後,月職能階段以5階(8,869元)為最高,4階次之(少2,096元),3階最少(少3,775元)。世代初始年月職能階段以5階(8,885元)為基準,4階次之(少4,611元),3階第三高(少4,737元)。五年內職能階段變化以有進有退(1,129元)為基準,只進階最高(多1,008元),維持階次之(多818元),只退階最低(少243元)。結論:精神專科醫院住院病友職能治療階段在工作復健獎勵金方面具有鑑別度。

Purpose. (1) To understand the distribution of progression, maintenance, regression, progression and regression of occupational therapy stage for patients from 2017 to 2021; (2) To compare the differences of average annual work rehabilitation bonus in the monthly functional stage within five years, the initial functional stage of a generation, and the five-year stage changes of occupational therapy. Methods. Research data came from the five-level multi-functional database and the work rehabilitation bonus database for inpatients received community rehabilitation in a psychiatric hospital of central Taiwan from January 2017 to December 2021. There were 42,192 person-months (812 patients) recruited as study sample for research purpose (1) and 41,785 people (991 patients) recruited as study sample for research purpose (2). Results. During the 5-year period, the progression rate was 25.6% (averagely spent 1.7±1.2 years), the maintenance rate was 11.0% (averagely spent 4.9±0.4 years), the regression rate was 38.1% (averagely spent 2.1±1.3 years), and the progression and regression rate was 25.3%. In terms of bonus, the level 5 of monthly function had the highest (NT$ 8,869), followed by level 4 (NT$ 2,096.3 less), and level 3 is the least (less than 3,775 yuan) after adjusting for confounding factors; the level 5 of cohort initial function had the highest (NT$ 8,885), followed by level 4 (NT$ 4,611 less), and level 3 (NT$4,737 less); when compared to progression and regression group (NT$1,129) of functional changes, the regression group had the lowest (NT $243 less), the progression group had highest (NT $1,007 more) , followed by maintenance group ($818 more). Conclusion. The stage of occupational therapy are evidenced for its discrimination in terms of work rehabilitation bonus for patients in the psychiatric hospital.
