摘要

供應中心是院內感染管制極重要的一環，所提供滅菌物品的品質，攸關醫療品質及病人成染之安全問題。於2008年1月8日至22日抽查滅菌包400件，有59件包裝不完整，不完整率為14.75%。重複處理包裝不完整的滅菌物品不僅耗費時間、人力，增加醫療成本，亦使服務品質降低，故引發改善動機。本專案主要目的在降低滅菌包包裝不完整性之情形。由現況分析得知造成滅菌包不完整的問題為人員缺乏滅菌包裝相關知識、廠商供應包布材質不良及缺乏包裝作業指引。藉由制定滅菌包包裝規範、安排物品包裝在職教育、與相關部門進行會議討論等措施，滅菌包不完整率由14.75%降為1.5%、人員物品包裝項目知識正確率由40%提升至92%、包布不良率由22.5%降至8.75%及能依循包裝作業指引完整率達92%，其專案成效顯著，不僅解決單位困擾，也提升醫療品質。

The CSR (Central Supply Room) is a very important unit to infection control in the hospital. The quality of sterilization closely correlates with safety and quality of care. From January 8, 2008 to January 22, 2008, we inspected by random sampling on 400 sterilization packages. We found that there are fifty-nine incomplete sterilization packages. The incomplete rate was 14.75%. Repeatedly dealing with incomplete sterilization packages not only waste time and manpower but increase the cost of the hospital. It also decreases the medical care quality. Therefore we have to increase our quality of care. The purpose of this project was to reduce sterilization packaging incomplete. According to the present analysis the reasons for giving incomplete sterilization packages were lacking the packaging instructions, knowledge of packaging sterilization packages and the poor quality of the materials. After establishing the rules of packaging process and educating our stall's and discussing with persons of related departments, the rate of incomplete sterilization packages was from 14.75% drop to 1.5%. The staffs' knowledge score of packaging was from 40% up to 92%. The rate of inferior cloth was from 22.5% drop to 8.75%. The project had a significant effect on providing complete sterilization packages. It solved the problem of our unit but also increased the quality of care.
