摘要

本文敘述一位中年口腔癌末期併發多重轉移男性病人的瀕死期照護過程，照護期間為2008年3月4日～2008年3月17日。我們針對個案腫瘤傷口出血、疼痛問題，給予適當的護理，緩解其生理的苦痛；建立信任關係後，協助個案及家屬面對預期性哀傷，包括引導他們相互表達愛，以繪本、畫圖協助個案幼子們表達感受等護理措施，促使個案與家人正向看待及接受即將來臨的死亡。同時，與安寧團隊進行全家照護，使個案身、心、靈等需求獲得滿足，照會並協同院牧與社工預備遺族輔導，個案於生命的最終時刻能劃上無遺憾的句點。分享此個案照顧經驗，作為護理同仁照護口腔癌末期病患及其家屬之參酌，期能達病人善終和更好的護理。

This report described the nursing experience of dying care to a mid-aged male with terminal phrase of oral cancer combining multiple metastases. The nursing care duration was from March 4th to March 17th, 2008.
We identified continuously health problems as tumor wound bleeding and pain; provided suitable interventions to release the patient's physical distress. After establish trust relationship with the case and his family, we assisted him and his family deal with the anticipatory grief of illness positively and accepted unavoidable death. Induced them speaking out ”love” to each other. Using picture books and painting to help his children realized dying and expressed how they feel about their dad at dying. Meanwhile, we and our hospice team provided whole family care, the patient was fulfilled physical, psychological, social and spiritual needs. We consulted with pastors and social worker preparing bereavement care for the family. Finally, the patient died in peace surrounding by his lovely family at our ward.
We expected our experience of this case as a reference for peer nursing staff to care oral cancer patients in terminal phase and their families for the patients' good death, and better care.
