摘要

當核心家庭之支柱成為家庭經濟支出、情感負荷的最大壓力源時，家庭會因衝擊而陷入危機，本文即是描述照顧這樣一個家庭之經驗。在一家之長病重住院成為呼吸器依賴個案後，此核心家庭困支持系統不足，無法面臨接踵而來的經濟及照護壓力，使家庭瀕臨瓦解。作者應用家庭調整與適應反應模式進行評估，發現個案家庭面臨家中成員溝通不良、家庭運作過程紊亂、家中成員身心疲潰等健康問題。因此以家庭為照護中心，讓成員彼此坦白溝通、幫助尋求社會資源，協助在家庭需求、能力及意義間恢復平衡，並在個案死亡後家庭成員能坦然展開新生活，成功幫助個案家庭度過危機。冀盼與護理同仁分享此照護經驗有助於臨床協助病患家庭渡過疾病過程危機，邁向正向適應。

When an important pillar of the nuclear family becomes the greatest source of pressure, economic costs, and el11otional loads for the family, he will no doubt make a large impact, even crisis, on the family. This article describes the experience of caring a family in crisis. After the pillar-father becoming seriously ill and ventilator-dependent, the nuclear family became nearly collapsed because of insufficiency of the supporting system to face the ensuing pressure of economics and heavy care burden. Through assessing the family with the adjustment and adaptation response model (FAAR), we identified that they faced several problems, including poor communication, interrupted family processes, and health problems in family members, such as physical and mental burnout. We provided family-centered care plan not only assists the members to communicate with each other honestly and find social resources but also assists in restoring the balances between the family needs, capacities and meanings. This successfully helps the family through the cri sis and assists them to start a new life comfortably. Sharing this experience with clinics may be useful for them to help the families facing the disease process and going through the cri sis towards a positive adaptation.
