運用賦權概念照顧一位思覺失調症病人之護理經驗
Applying the Concept of Empowerment in Nursing Care for a Patient with Schizophrenia
摘要
本文為照護一位缺乏病識感、服藥遵從性差及自我評價低的思覺失調症病人之護理經驗。筆者於2020/1/16～2020/3/17藉由觀察及會談收集資料，並依：生理、心理、智能、社會、靈性等進行整體性護理評估，歸納主要問題有：感覺知覺紊亂、知識缺失／對疾病及藥物與長期性低自尊。運用賦權概念規劃八次結構式疾病管理課程並製作手冊，提升對症狀認知與服藥遵從性；教導幻聽因應策略；執行促進人際互動與自我肯定訓練。結果顯示可增進病人幻聽因應技巧及改善服藥態度，達成疾病、藥物及生活層面之自我管理，自我肯定量表從23分提升至40分，順利回歸家庭及適應。期望此經驗分享，更重視讓病人參與治療計劃，改善傳統衛教模式，促進病人疾病自我管理能力及自我價值感。

This study presented the nursing experience of caring for a schizophrenia patient had no insight, exhibited poor medical compliance, and negative self-evaluation. The period of nursing care was from January 16th to March 17th, 2020. The data were collected by observations and interviews, and the holistic nursing assessment was conducted in accordance with five major aspects. The major nursing problems found during the hospitalization were: perceptual feeling variation in hearing hallucinations, a lack of knowledge, and long-term low self-esteem. Using the concept of empowerment to design eight structured disease management courses and made a handbook to improve insight and medication compliance, and strategies in responding to auditory hallucinations; social skills training and self-affirmation training to promote interpersonal interaction. The results show using the concept of empowerment can improve the patient's auditory hallucination coping skills and attitude of taking medicine to achieve self-management of diseases, drugs and lifestyle. The score of self-affirmation scale from 23points increased to 40points and successfully return to family and community adaptation. This experience will be used to provide care colleagues in the future and take good care of such this case. To attention will be paid to allowing patients to participate in the treatment plan, improving the traditional health education model, and promoting patients' disease self-management ability and self- esteem.
