Backgrounds: Taipei has become an aging society since 1993. Home care is the earliest and most well-developed service delivery in community-based elder care. The aim of this study was to explore the home care services in Taipei City in the year 2002 and 2003, and to promote agencies community-oriented geriatric care. Materials and Methods: Home care agencies in Taipei is required to report its service data to the Taipei City Government on a regular basis. In 2004, with proper authorization we collected the service data in 2002 and 2003 for study. Data included the cases numbers under services, physician visits, and nursing visits and were stratified according to the hospital status (medical center, regional hospital, local hospital and independent home care nursing institute). Ratios of physician and nursing visits were calculated and compared serially. Results: In total, 25 home care agencies (6 in medical centers, 10 in regional hospitals, 8 in local hospitals and one free standing home care nursing agency) were enrolled for study. In 2002, 3919.7 subjects received home care services with an average of 3.5 physician visits and 16.8 nursing visits per year. Mean physician visits per patient in medical centers, regional hospitals, local hospitals and independent home care nursing agency were 3.7±1.7, 3.7±1.6, 4.3±1.4, and 1.0 per year respectively, and the mean of nursing visits per patient read respectively 17.7±2.4, 14.9±3.9,17.2±4.8, and 13.7 per year. In 2003, there were 4140.6 subjects implemented home care services with 3.7 physician visits and 17.0 nursing visits per year. The mean physician visits among medical centers, regional hospitals, local hospitals and independent home care nursing institute were 3.6±1.9, 4.0±1.3, 4.4±1.5, and 0.2 per year respectively, and the mean of nursing visits read 17.4±2.4, 17.4±2.7,17.3±1.1, and 15.7 per year respectively. The ratios of physician and nursing visits were significantly different among different settings in 2002 and 2003 (p=0.025 in 2002, p<0.001 in 2003). Conclusion: Though home care services in Taipei City are mainly hospital-based. However, in addition to sufficient physician visits, the National Health Insurance should subsidize the services of health professionals other than physicians and nurses so as to improve the quality of community-based long-term care for the elderly.
