中西醫治療肺炎病例討論

Management of Pneumonia by Integrated Chinese and Western Medicine: A Case Report

摘要

此病例爲80歲男性病患，過去病史有陳舊性腦中風，心房顫動，中度至重度的主動脈瓣閉鎖不全併心臟衰竭及慢性阻塞性肺病，診斷爲肺炎合併急性呼吸衰竭，在給予氣管內管插管、呼吸器支持與抗生素治療1個月後，由於臨床症狀與影像學檢查改善緩慢仍無法成功拔除氣管內管，而且住院過程中，也發生心跳徐緩與短暫性心室頻脈的危症。經由會診中醫，在中醫的辨證論治與整體評估後，使用益氣通陽宣肺與清化痰濁藥物治療，中西醫合併治療肺炎，在改善臨床症狀與影像學的表現後，得以拔管成功，6天後出院。

This 82 y/o man who has past history of old CVA, atrial fibrillation, aortic valve regurgitation with CHF and COPD, was diagnosed of pneumonia with acute respiratory failure. After endotracheal tube insertion with ventilator support and antibiotics were prescribed for about 1 month, the patient cannot be weaning from endotracheal tube based on sustained symptoms and results from image examination. Bradycardia and transient ventricular tachycardia were also noted during hospitalization. After the systemic evaluation and treatment by consulting doctors of traditional Chinese medicine, herbs of supplementing Qi and restoring Yang, and ventilating lung and resolving phlegm were prescribed for integrated Chinese and Western medicine for pneumonia with respiratory failure. Finally, the patient was discharged 6 days later with successful weaning after the improvement of symptoms and results from image examination
