本文乃運用行為修正理論,協助年輕男性病人面對首次血液透析之護理經驗。護理期間自2017年3月1日至3月11日,筆者以Gordon十一項功能健康評估,經觀察、會談及理學檢查等方式收集資料。確立護理問題:(1)體液容積過量。(2)知識缺失。(3)焦慮。因此病人為年輕男性,其需面對之角色壓力及因應措施與年長者不同,故筆者於護理過程中,考量個別需求,採行「行為修正」理論為介入措施,讓個案了解首次透析,所需具備之自我照護知識與技能,進而降低日後需面對長期洗腎之身、心理壓力,並透過建立病人持續自我監測的概念,以期返家後能落實自我健康管理。期望藉此護理經驗分享,提供護理人員,面對日後照護年輕洗腎病人時之依循,進而協助其能與疾病共存。
This report describes the care experience of assisting young male patients with first hemodialysis. The care period was March 1st to 11th, 2017. The author applied Gordon's 11 functional health patterns, observations, interviews, and physical assessments to collect data for analysis. The following health problems were identified for the patient: fluid volume excess, knowledge deficit, and anxiety. Considering the role stress and coping strategies of young adults were different with the elderly, the author adopted the theory of Behavior Modification Strategies to equip patients with self-care knowledge and skills before the first hemodialysis treatment, aim to reduce both physical and psychological stress and establish self-monitoring concept for further self-care management. This report may serve as a reference for nurses when providing nursing care for young hemodialysis patients and helping them to achieve a balance between normal life and disease control.
