運用醫療團隊資源管理提升糖尿病人衛教指導正確率

衛教指導不確實，導致病人傷害事件。良好衛教指導可預防合併症產生。本專案運用團隊資源管理提升糖尿病人衛教指導正確率。經現況分析，衛教指導正確率僅達58.2%，分析原因：（1）對團隊資源管理模式不了解、（2）缺乏糖尿病衛教指導教育訓練、（3）未建立糖尿病衛教指導標準流程、（4）缺乏糖尿病輔助教學工具、（5）未建立糖尿病衛教指導監測制度。經介入措施（1）舉辦團隊資源管理及糖尿病課程、（2）制定衛教指導標準作業書、（3）衛教單張及海報、（4）衛教指導手冊、（5）增購書籍及教材、（6）製作足部護理及運動指導影片、（7）轉介衛教師、（8）訂定監測機制。糖尿病人衛教指導正確率由58.2%提升至98%，提供正確衛教指導，促使病人落實健康管理，以提升日後自我照護成效。

Incorrect health care guidance led to patient injury. Good health education can prevent comorbidities. This project uses team resource management to improve the accuracy of health education guidance for people with diabetes. According to the current situation analysis, the correct rate of health education guidance is only 58.2%. The reasons are: (1) not knowing the team resource management mode, (2) lack of diabetes health education guidance training, (3) no standard process of diabetes health education guidance. The correct rate of health education guidance for diabetic patients was increased from 58.2% to 98%, providing correct health education guidance to urge patients to implement health management to improve the effectiveness of self-care in the future.
