提升移植病人手部衛生正確率

Improve The Accuracy of Hand Hygiene In Transplant Patients

摘要


醫療感染的發生重要傳染媒介是雙手,移植病人免疫力低下,若能執行正確手部衛生,可降低感染機會。本專案旨在提升移植病人手部衛生正確率,確立原因有:病人對於手部衛生觀念及步驟不瞭解,認為只要有洗手就乾淨、護理人員在手部衛生衛教指導內容僅簡單口述,多數無實際操作洗手步驟及觀察病人洗手行為、手部衛生之重點,病人無法在短時間內吸收,僅單張文字贅述,無法具體觀察手部衛生正確性。經專案實施:手部衛生步驟查檢表、製作手部衛生衛教影片、運用洗手步驟圖、病室洗手台壁式泡沫洗手液及螢光劑檢測手部衛生乾淨度、進行手部衛生團體衛教。藉由專案推行使移植病人手部衛生正確率由61.87%提升至94.64%,以達專案之目的。

An important vector for medical infections is the hands. Transplant patients have low immunity. If proper hand hygiene is performed, the chance of infection can be reduced. This project aims to improve the accuracy of hand hygiene of transplant patients. The reasons are: patients do not understand the concepts and procedures of hand hygiene, and think that as long as they wash their hands, they should be clean. There is no actual hand-washing steps and the focus of observing the patient´s hand-washing behavior and hand hygiene. The patient cannot absorb it in a short period of time. Only a single text is repeated, and the correctness of hand hygiene cannot be observed in detail. Implemented by the project: hand hygiene step checklist, production of hand hygiene education videos, use of hand washing step diagrams, ward wall foam hand sanitizer and fluorescent agent to test hand hygiene cleanliness, conduct hand hygiene group hygiene education .Through the implementation of the project, the accuracy rate of hand hygiene of transplant patients was increased from 61.87% to 94.64% to achieve the purpose of the project.
