照顧一位化膿性汗腺炎個案之護理經驗
Experience of Nursing Care a Patient with Hidradenitis Suppurativa
摘要
本文描述一位罹患化膿性汗腺炎多年的男性，因雙腋窩、臀部及雙大腿後側皮膚受損化膿嚴重，疾病長期影響造成個案心情低落且不敢與他人接觸，筆者運用Gordon十一項健康評估，確認個案有皮膚完整性受損、疼痛及無望感等護理問題。於2019年6月5日至2020年1月8日門診照護期間，採個別性照護並提供皮膚照護技巧及疼痛控制等措施，以提升生活品質。另外，鼓勵個案正向面對治療並突破社交障礙，達成個案走出戶外結交朋友的目標。此外，過程中發現院內無設立化膿性汗腺炎或皮膚相關之病友會，且因化膿性汗腺炎之病人較為罕見，使病人難以取得相關知識，故建議院方可成立定期性講座或製作相關衛教單張，期許未來可提升化膿性汗腺炎病人之照護品質。

This case report describes a man who had suffered from hidradenitis suppurativa for many years. He was unable to be correctly diagnosed for a long time, causing serious skin lesions over his armpits, buttocks and bilateral posterior thighs. The situation depressed the patient and caused him being afraid to contact with others. We use Gordon 11 Function Health Patterns including observation, physical assessment, interviews, and medical record queries to confirm that the case had nursing problems such as impaired skin integrity, pain, and hopelessness. During in the period of outpatient department from June 5, 2019 to January 8, 2020, individual nursing care including skin care skills and pain control was provided to improve his quality of life. In addition, after building trust with the case, we encouraged him to face the treatment positively and achieve the goal to make friends with others.
