摘要

本文為照護一位年輕男性，酒駕自撞導致脾臟破裂合併焦慮之急診照護經驗。照護期間自2022年11月23日01：30至04：05，運用整體性護理評估收集資料，確立有體液容積缺失、急性疼痛與焦慮之健康問題，因突逢意外需緊急手術，個案面對多重壓力產生焦慮，在急診崩潰之畫面震懾筆者，運用同理心主動關懷、陪伴並引導抒發內心的衝擊與感受，聯繫家屬前來陪伴，使個案能獲得家人的支持與情緒宣洩，協同醫療團隊說明病程進展的治療方向，成功緩解個案之焦慮，其順利接受手術，期望藉此分享照護經驗於類似個案之參考，了解即使當個案處於腹部創傷急性期，亦能發現心理層面之問題，提供整體性的評估與照護，迅速穩定病況、降低焦慮以儘早接受進一步的治療。

This paper describes the emergency care experience of a youn with g manspleen rupture and anxiety caused by a drunk driving crash. During the period of care, from 1:30 am to 04:05 am on November 23, 2022, the data collected by holistic nursing assessment was used to establish that there were health problems such as fluid volume loss, acute pain and anxiety, emergency surgery was required due to sudden accidents, anxiety caused by multiple pressures, and the picture of collapse in the emergency room was shocked the author. We took the initiative to care for, accompany and guide the expression of inner impact and feelings with empathy, contacted family members to provide a support system, so that the case could be cathartic, coordinated with the medical team to explain the treatment direction of the course of the disease, successfully relieved the anxiety of the case, and successfully underwent surgery. I hope to share my care experience for reference in similar cases, and understand that even when the case is in the acute stage of abdominal trauma, It can also detect psychological problems, provide holistic assessment and care, quickly stabilize the condition, reduce anxiety and receive further treatment as soon as possible.
