中醫治療異位性皮膚炎之病例報告

Treatment of Atopic Dermatitis Using Traditional Chinese Medicine : A Case Report

摘要


全台灣民眾在2020年就有超過152萬人因受濕疹所擾而就診。2003年到2012年,台灣有30.5%的異位性皮膚炎患者曾經使用中醫藥,而且呈現逐年遞增的趨勢,可見傳統中醫藥在異位性皮膚炎的治療上益形重要。異位性皮膚炎可在任何年齡發病,大約四分之一的病人在成年後才發病。異位性皮膚炎的特徵是劇烈瘙癢和反復出現的濕疹病變,並且病人的臨床表現差異頗大。中醫認為異位性皮膚炎的病因病理多由於病人體質較容易感受邪氣,風、濕、熱阻於肌膚所致。本臨床案例報告一位49歲已婚女性之異位性皮膚炎患者,成年後發病,皮膚癢疹腫熱反覆,接受西醫皮膚科治療多年未癒,轉由中醫治療的歷程。進而分析患者皮膚癢疹的病因病機與運用中藥方劑治療之成效。

In 2020, more than 1.52 million people in Taiwan visited a doctor due to eczema. From 2003 to 2012, 30.5% of patients with atopic dermatitis in Taiwan used Chinese medicine, and the trend is increasing year by year. It can be seen that traditional Chinese medicine is more important in the treatment of atopic dermatitis. Atopic dermatitis can develop at any age, and about a quarter of patients do not develop it until they reach adulthood. Atopic dermatitis is characterized by severe itching and recurring eczema lesions, and the clinical manifestations of patients are quite different. Traditional Chinese medicine believes that the etiology and pathology of atopic dermatitis is mostly caused by the susceptibility of the patient, and wind, dampness, and heat block the skin. This clinical case reports a 49-year-old married woman with atopic dermatitis. She developed symptoms of prurigo, swelling and heat recurring in adulthood. She received Western medical dermatology treatment for many years but in vain, then turned to TCM treatment. We analyze the etiology and pathogenesis of patients with prurigo and the effectiveness of treatment with traditional Chinese medicine.
