癌症惡病質之中醫藥治療文獻回顧

摘要

惡病質是一種高代謝狀態的慢性發炎反應，為多因素與多器官之症候群，主要症狀為體重減輕、食慾不振與肌肉消耗，常為癌症末期造成死亡的原因，目前並無特殊藥物治療。本文以「惡病質」為關鍵字搜尋「中國期刊全文數據庫」與「華藝中文電子期刊資料庫」，納入2010至2021年中醫藥治療癌症惡病質之臨床案例論文共26篇，以補脾益氣最多共有12篇，常見方劑為參苓白朮散；其次為益氣養血或益氣養陰共有7篇；其餘另有脾腎雙補3篇及扶正祛邪法4篇。癌症惡病質病因複雜，本文提供癌症惡病質治療之選方參考，中醫辨證論治個人化專治，且中西醫合療可減輕癌症治療副作用並增加療效。期待後起醫家能有更多文獻發表，使中醫藥治療癌症惡病質更多樣性。

Cachexia is ahypercatabolic statein chronic inflammatory response. Loss of weight and appetite with muscle wasting are commonsymptoms in these patients.It is a multifactorial and multi-organ syndrome, and it is one of the main causes of death in late stages of cancerpatients. To date, there is no specific treatment for cancer cachexia. We reviewedliteratures about cachexia ofChina Academic Journals Full-text Database, CJFD, and finally included 26 articles. The main treatment principle of these articles isfacilitating the GI tract motility and absorption(BU-PI-YI-QI), and the most common decoction is SHEN-LING-BAI-ZHU-SAN; the second principle is nourishing qi, blood or yin(YI-QI-YANG-XIE or YI-QI-YANG-YIN). Others are improving the function of GI tract and kidney(PI-SHEN-SHUANG-BU), and enhancing the immune system with eliminating pathogens(FU-ZHENG-QU-XIE).The TCM literatures in our study will benefit clinical treatment and research of cancer cachexia in thefuture.
