本研究目的在探討精神病患參加服藥團體之歷程。研究對象為某醫學中心七名精神病患。研究者擔任團體領導者,團體採結構方式,每週舉行一次六十分鐘共進行八次。全程現場錄音與紀錄,並將團體過程轉謄為逐字稿,以質性研究內容分析法,進行內容編碼分析。結果發現締結同在、覓求調適、激勵共勉的互動歷程,成員能分享主體經驗,並藉由比較他人與自己的經驗,加深自己對服藥的領會。團體中激發的核心內涵,分別為解開心結及儲備能量。長期服藥的折磨、副作用的衝擊、疾病附加的壓力為主要的心結。接納生病與服藥的事實,聯結服藥、生活與希望是帶給病患持續服藥的能量。研究結果可提供臨床強化病患服藥的動力。期待本研究可作為未來架構精神病患服藥團體操作型式及衛教內涵修正的參考。
The purpose of this research was to discuss the process of participation in a medication-taking group of psychotic patients. The group contained seven psychotic patients in total, in stable mental condition, from a certain medical center. The researcher served as a leader of the structured group. Total eight sessions with each for sixty minutes every week. The whole process was recorded and translated, and then analyzed and coded using the qualitative method. The result was that by the process of interaction with each other, pursuing adaptation and inspiration, the group members could share subjective experiences and reinforce their own understanding of medication taking by comparison with others. There are two important therapeutic meanings of group operation: one is the untying of the twisted mind, and the other is the storing of energy. The first involves dealing with the suffering and side-effects caused by long term medication-taking, and the added pressure of the disease. The other involves enabling group members to accept the truth of the disease and the medication taking, and to connect with life and hope. The result could provide the key to the reinforcement among patients of the need to take medication. We hope this research will serve as a reference in the future revision of structured groups of medication-taking psychotic patients.
