社區精神衛生護理能力-建構自我

The Construction of Essential Competence of Community Psychiatric Mental Health Nursing-Self Construction

摘要


本研究目的為發展精神衛生護理人員於社區從事「協助病患建構自我」之能力範疇與操作性定義。本研究採行動研究法,基於李等(2008)之研究有關「協助病患建構自我」結果進行社區實證護理,由照護過程的實證來省思及確認「建構自我」護理能力的範疇與操作性定義。研究對象為實務工作者亦為研究者,有三位為三所醫院之臨床護理人員(包含一位護理主任、一位護理長及一位精神科資深護理人員),另一位為學校的護理教師,資料分析採內容分析法,分析照護個案中互動記錄內容所呈現之行為操作項目,並依據分析之結果省思及確認實務能力之範疇與操作性定義。研究結果顯示「建構自我」之能力範疇包含三個主要項目,並對其發展行為操作性定義,分述如下:(一)協助個案探索自我。(二)促進個案肯定自我。(三)強化個案動能。此結果可做為進一步發展實務能力的培訓與認證制度之參考,以提昇社區精神衛生護理的照護品質,並促進本實務能力在臨床上的靈活運用。

The purpose of this study was to develop the scope and operational definitions of ”self construction” essential competence of community psychiatric mental health nursing. Method: The action research method was applied in this study. Based on Lee's research, ”Exploring the Clinical Competency of Psychiatric Nurse in Continuous Nursing Care of Outpatient Department”, the self-construction was confirmed by clinical care process, focus groups, and expert discussions. Subjects were both clinical participants and researchers: three clinical nurses from three hospitals (including one nursing director, one head nurse, and one advance clinical nurse) and one nursing faculty. Content analysis was used to analyze the process recording. Results: The scope and operational definitions of ”self construction” essential competence were identified as the following three main items: (1) Assisting patients to explore themselves. (2) Assisting patients to accept their own emotions and to value themselves highly. (3) Supports in patients' motivation enforcement. Conclusion: The results of this study could serve as a basis guideline for developing model of training and accreditation of essential competence for community mental health nursing. The results might enhance the quality of care for community health caring and promote the ability of practical application of clinical practice.
