本研究的目的為發展護理人員於社區協助精神病患建立自我管理之實務能力範疇與行為操作定義。本研究採行動研究法,以自我管理之理論與協助復健期精神病患提升自我管理之研究結果,進行社區個案照護,以照護實證資料進行內容分析,發展出護理人員「協助病患自我管理」的實務能力範疇與行為操作定義。研究結果支持「協助病患自我管理」之能力範疇含:(一)評估個案自我管理狀況;(二)確定困難障礙及問題順序;(三)共同決定目標;(四)探索方法;(五)執行策略;(六)增強與評價自我管理策略等六項目。本研究發展結果可提升精神病患自我管理技能之護理策略,並可做為鑑定護理人員實務能力之參考。
The purpose of this study was to develop the scope and performance of nursing essential competence for promoting self-management skills of patients with psychosis. Method: The action research method was applied in this study. Based on the theories of self-management and studies of self-management programs on the outpatients with psychosis in the rehabilitation stage, the competences of promoting patients self-management skills were confirmed by nursing process, focus groups, and expert discussions. Subjects were both clinical participants and researchers including two clinical nurses providing directly community care and two nursing faculty. Content analysis was used to analyze the process recording. Results: The scope and performance of ”promoting patient's self-management skills” clinical competency were identified as the following six main items: 1. Assessing the patient's ability to monitor one's condition inherent in living with a chronic diseases. 2. Recognizing and identifying problems that patient experiences. 3. Pacing, planning and prioritizing the goal of self-management and changes in patient's daily life. 4. Making an action plan. 5. Practicing the self-management strategies. 6. Evaluating and increasing patient's confidence in the ability of making life-improving changes. Conclusion: The results of this study could serve as a basis guideline for nurses to promote the patients' self-management skills, and to develop a model of training and accreditation of essential competency for psychiatric community mental health nursing.
