建構社區精神衛生護理能力－疾病管理
The Construction of Essential Competence of Community Psychiatric Mental Health Nursing-Illness Management
摘要
本研究目的為建構社區精神衛生護理人員的個案疾病管理能力之範疇與內容，以提昇護理實務能力。本研究採行動研究法，由資深護理人員臨床照護的實證來省思及確認「疾病管理」護理能力的範疇與操作性定義。研究對象為實務工作者亦為研究者，為某精神專科醫院之四位臨床護理人員（包含一位護理主任及三位資深護理人員），資料分析採內容分析法，分析照護個案中互動記錄內容所呈現之行為操作項目，並依據分析之結果省思及確認實務能力之範疇與操作性定義。研究結果顯示「疾病管理」之護理能力範疇包含三個主要項目，並對其發展行為操作性定義，「疾病管理」之能力範疇為：（一）評估疾病對個案日常生活的影響程度。（二）協助建立病識感。（三）促進適應能力。此結果可做為進一步發展實務能力的培訓與認證制度之參考，以提昇社區精神衛生護理的照護品質，並促進本護理能力在臨床實務上的運用。

The purpose of this study was to develop the scope and operational definition of ”illness management” essential competence of community psychiatric mental health nursing. Method: The action research method was applied in this study. The illness management was confirmed by clinical care processes and expert discussions. Subjects were both clinical participants and researchers including four clinical nurses (include one nursing director and three senior clinical nurses). Content analysis was used to analyze the process recording. Results: The scope and operational definitions of ”illness management” clinical competency were identified as the following three main items: (1) Assessing illness effective level in daily living. (2) Assisting the client to establish insight. (3) Promoting adaptation capability. Conclusion: The results of this study could serve as a basis guideline for developing model of training and accreditation of essential competency for community psychiatric mental health nursing caring. The results might enhance the quality of care for community health caring and promote the ability of application in clinical practice.
