運用PDCA循環提升美沙冬服藥出席率之專案

本專案主旨在提升藥癮病人美沙冬服藥出席率。統計2014年1月至11月美沙冬平均服藥出席率86.98%，每月未達衛生局目標，經資料分析，確認問題：（一）病人未能完全遵從療程、（二）工作人員未關注出席率低病人、（三）單位沒有出席管理規範、(四)工作人員未給予病人充足衛教。故成立專案小組，運用PDCA循環，擬定解決方案：（一）建構電話關懷機制；（二）設立就診前置作業；（三）明訂服藥出席率級距原則。結果顯示美沙冬服藥出席率由86.98%增加至90.63%，顯示專案措施之可行性與有效性，亦獲得附加效益留置率由89.19%提升至92.84%，期許本專案作為同儕借鏡，進而增加藥癮照護品質。

The purpose of this project was to improve the attendance rate of drug-addicted patients who accept the methadone treatment. Statistics showed that the patients receiving the methadone maintenance treatment whose monthly attendance rate at average during the period from January to November in 2014 was 86.98%, which did not reach the goal set by the Department of Health. The problems included that (1) patients failed to comply with the course of treatment, (2) nursing staff failed to concern about the patients showing low attendance, (3) nursing units lacked the management practices for urging attendance, and (4) staff failed to provide the patients with sufficient health education. Therefore, by applying the Plan- Do-Check-Action (PDCA) cycle, the project team conducted improvement strategies as follows: (1) constructing a follow-up telephone care mechanism, (2) implementing a pre-visit planning, and (3) stipulating the principles based on attendance scale. The results showed an improvement of patients’ attendance accepting the methadone maintenance treatment from 86.98% to 90.63%, and it proved the feasibility and validity of this project. Additionally, the patients’ retention in treatment was elevated from 89.19% to 92.84%. This project could serve as a reference for nursing professionals in providing good quality of drug addiction care.
