摘要

背景：護理除了身、心的照護以外，應包含靈性的層面。當精神科病人靈性需求未獲滿足時，可能造成服藥不遵從、住院天數及再住院率增加等問題。臨床上護理人員執行靈性層面評估，只聚焦於宗教信仰類別，未能深入評估病人內在感受。專案目的：提升精神科病房護理人員評估病人靈性需求能力。執行過程：經分析，發現護理人員無法深入評估靈性的問題在於，缺乏靈性需求評估指引、缺乏靈性需求評估教學工具、缺乏相關之教育訓練課程、對靈性需求評估不了解、及缺乏稽核制度等。本專案目標為提升精神科護理人員靈性需求評估能力，其方案制定為「靈性需求評估引導語指引」，製作「靈性需求評估教學光碟」，並「舉辦在職教育訓練」，設立「稽核制度」。結果與建議：專案實施後，護理人員靈性需求評估能力可提升至95.5%。本專案可提升精神科護理人員靈性需求評估能力，建議可做為日後護理人員在臨床提供病人靈性需求評估之參考。

Background: Nursing activities encompass the physical and psychological care of patients as well as the fulfillment of their spiritual needs. Unattended spiritual needs of patients in psychiatric wards can cause them to demonstrate problems such as medical noncompliance and increases in the length of stays and likelihood of rehospitalization. In current practices, clinical nurses mainly assess the spiritual needs of patients on the basis of their religions and usually overlook the in-depth assessment of their feelings. Objective: To improve psychiatric nurses' competence to assess the spiritual needs of patients. Methods: Analysis revealed that the inability of nurses to perform in-depth assessment on patients' spiritual needs was caused by the absence of assessment guidelines, teaching tools, training courses, and auditing mechanisms as well as their lack of understanding in spiritual needs assessment. To improve nurses' competence to assess the spiritual needs of patients, this project created a set of spiritual needs assessment guidelines and a teaching program (stored in an optical disc), hosted in-service training, and established a auditing mechanism. Results and suggestions: Psychiatric nurses' competence to assess patient spiritual needs was improved to 95.5%, verifying the feasibly of the implemented measures. The project can serve as a reference for clinical nurses to assess the spiritual needs of patients.
