瑜珈對住院精神病人疲倦、睡眠、焦慮、憂鬱之成效

Effects of Yoga on Fatigue, Sleep, Anxiety, and Depression Among Psychiatric Inpatients

摘要


目的:探討瑜珈於住院中精神病人的疲倦、睡眠、焦慮、憂鬱改善之成效。方法:本研究採用類實驗設計,方便取樣中部某醫學中心急性精神病房病人共100位,兩組皆進行藥物治療及病房常規照護,另外實驗組(n = 50)接受每週5天,每次30分鐘的瑜珈活動,對照組(n = 50)未介入。研究工具包括「李氏疲倦量表」、「李氏一般睡眠障礙量表」、「貝克焦慮量表」、「貝克憂鬱量表」分別測量受試者的睡眠、焦慮及憂鬱程度。測量時間有三次,分別為介入前、介入10次(二週)後及介入20次(四週)後,資料分析應用獨立樣本t及概似化估計方程式(generalized estimating equations, GEE)檢驗。結果:實驗組接受瑜珈活動10次後,在疲倦(p < .001)及睡眠(p < .001)顯著優於對照組,GEE結果呈現兩組於疲倦及睡眠之表現具顯著差異(B = -1.56, p < .005、B = -0.74, p < .006),而介入20次後呈現實驗組於疲倦及憂鬱之改善顯著優於對照組(B = -1.17, p <. 048、B = -7.19, p < .018)。結論:精神病人參與瑜珈活動,可改善疲倦、睡眠、焦慮及憂鬱程度,此研究成果可提供未來相關研究及護理實務介入措施的依據及參考。建議除規則服藥外,可在專人的引領下,提供適合的瑜珈活動。

Objective: To explore the effects of yoga on fatigue, sleep, anxiety, and depression in psychiatric inpatients. Methods: This quasi-experimental study recruited 100 inpatients from the acute psychiatric ward of a medical center in the central Taiwan through a convenient sampling procedure. Patients were divided into an experimental group (n = 50), which received a 30-minute yoga session five times per week for 4 weeks along with the standard medication and routine care, and a control group (n = 50), which did not receive yoga intervention. The Lee Fatigue Scale, General Sleep Disturbance Scale, Beck Anxiety Inventory, and Beck Depression Inventory were respectively used for measuring the patients' fatigue, sleep quality, anxiety, and depression levels before, 2 weeks after, and 4 weeks after the intervention. Data were analyzed using independent-samples t tests and generalized estimating equations (GEEs). Results: After 10 yoga sessions, the experimental group had significantly lower fatigue (p < .001) and higher sleep quality (p < .001) than the control group. The GEE results demonstrated fatigue and sleep in two groups (B = -1.56, p < .005, B = -0.74, p < .006). After 20 yoga sessions, the experimental group had significantly lower fatigue and depression than the control group (B = -1.17, p < .048, B = -7.19, p < .018). Conclusion: Yoga can reduce fatigue, improve sleep, and alleviate anxiety and depression in psychiatric inpatients. The results of this study can serve as a reference for developing intervention measures in nursing practice. We recommend the administration of appropriate yoga intervention under the guidance of a qualified yoga teacher, in addition to standard medication.
