摘要

復元導向照護受到全球精神衛生照護之關注並成為精神心理健康照護之目標。精神復元被定義為精神病人與疾病生活獨特的經歷或社區精神衛生照護之成果。社區精神病人之照護需求滿足與其復元有顯著正相關，但文獻鮮少以病人為中心，依此群體需求而客製化之復元照護方案。台灣刻正推動社區化照護政策的關鍵時刻，有需要準備並拓展護理師所需知能，並認證為社區精神衛生護理師，以實踐促進社區精神病人復元照護目標。本文描述筆者國科會專題研究計畫所發展由社區精神衛生護理師主導之需求導向復元個案管理方案，對社區精神病人之需求滿足、復元、希望及信守服藥皆有顯著成效，內容包括四個主要部份：精神復元定義與社區精神衛生護理師角色、精神復元照護的核心要素、實證復元照護方案－以需求裁量復元個案管理為例、以及社區精神衛生護理師所需的實務能力及認證。

Recovery-oriented care has attracted considerable attention globally and is considered to be the objective of mental health care. Community-dwelling people with serious mental illness are more likely to recover if their needs are met; however, tailored, person-centered solutions for this population remain scarce. Taiwan is at a critical juncture in its implementation of community-based care policies. In particular, nurses should be officially trained to be community psychiatric nurses. Thus, this this article describes a novel recovery-oriented program led by community psychiatric nurses for community-dwelling people with serious mental illness. This paper covers the following topics: the definition of mental health recovery and roles of community mental health nurses, the core elements of mental health recovery care, the needs-tailored recovery model of case management, and the skills of community psychiatric nurses and corresponding certification systems that are required.
