摘要

瀰漫性血管內凝血常見於癌症、胎盤早期剝離、敗血性疾病、及毒蛇咬傷等。頭部外傷的患者也會併發瀰漫性血管內凝血，若於麻醉手術期間造成無法止血，將發生出血性休克的危象。此報告介紹一位頭部外傷患者於手術中出現出血不止，後經診斷為瀰漫性血管內凝血。藉此個案探討頭部外傷患者，若於手術麻醉期間出現瀰漫性血管內凝血，麻醉人員應如何處理此危象問題，並探討瀰漫性血管內凝血的致病機轉與進一步的傷害，作為後續治療參考。

The Disseminated intravascular coagulation (DIC) occured in malignant tumor, placenta abrupation, sepsis, snake bite usually, it also happen in head trauma cases. If the DIC develops in the operation period, it will induce hypovolemic shock due to the poor hemostasis. We report a case with DIC developed in the operation period due to head injury and discuss the pathwphysiologic mechanism of DIC which will provide some help for the managemet of DIC to clinician.
