照護一位心臟衰竭病患之護理經驗
Nursing Experience of Caring a Patient with Congestive Heart Failure
摘要
本文描述一位因心臟衰竭初次入院病患之護理經驗，照護期間為2009年11月2日～11月12日，運用觀察、會談、身體評估方式收集資料，以Gordon十一項功能性健康型態進行護理評估，確立個案有「氣體交換功能障礙」、「活動無耐力」、「焦慮」三項護理問題。護理過程中，教導個案有效深呼吸咳嗽以增進氣體交換率；擬定漸進式活動計劃改善個案活動無耐力情形；鼓勵主要照顧者共同參與照護計畫，提供心臟衰竭相關知識，減輕其內心焦慮；依個案活動耐力及個別性擬定適切的復健運動計劃，使其對疾病預後控制產生正向肯定，進而增強其對心臟衰竭的認知及出院後居家心臟復健的執行意願。最後個案因心臟衰竭致呼吸困難等生理不適症狀獲得改善轉至普通病房。期望藉此照護經驗之分享，作為心衰竭病患全面性照護之參考依據，使得護理品質更為連續，以降低心臟衰竭病患反覆再住院率。

This report described the nursing experience of a patient admitted to the hospital with congestive heart failure for the first time. During the caring period between 2nd, November and 12th, November, 2009, methods such as Gordon's 11 Functional health patterns assessment. The nursing problems included impaired gas exchange, activity intolerance, anxiety. To improve the gas exchange, the patient was trained to practice effective deep breathing and coughing throughout the caring period. Progressive activity program was planned and implemented to alleviate activity intolerance. To relieve patient's anxiety, heart failure-related knowledge was provided and the primary caregiver was encouraged to participate in the treatment plan. An adequate rehabilitative exercise program was individually planned according to with patient's activity tolerance. Hence the prognosis was managed with positive attitude while the rehabilitation program was implementing. Subsequently, the heart failure-related knowledge and the willingness to practice rehabilitation programs were enhanced. As the improvement in symptoms of dyspnea attributed to heart failure, the patient was eventually transferred to a general ward. By sharing the nursing experience described in this article, the author anticipated to establish the principle of providing comprehensive healthcare and to lower readmission rates for the patients with congestive heart failure.
