協助一位腦中風病人脫離鼻胃管及氣切管之居家護理經驗

摘要

本個案報告是描述一位首次中風病人剛出院居家照護期間之護理經驗。護理期間自98年12月04日至99年06月30日，筆者運用身體、情緒、家庭及社會、經濟與靈性等五大層面進行整體性護理評估工具，以身體評估、會談、觀察、個別衛教指導、回覆示教、評值等技巧，收集資料加以整理分析後，確認個案有1、潛在危險性肺吸入；2、身體活動功能障礙；3、自我照顧能力增進準備度；4、照顧者壓力負荷過重等護理問題。在照護過程中，以健康促進的概念運用護理專業適時提供各種照護知識及可運用資源，把握復健的黃金時期，針對個案問題提供個別性照護計畫，給予信心及支持、協助個案改善自我照顧能力，提升居家更好的生活品質。期藉此特別的居家護理經驗與大家分享，提供日後居家護理同仁照護此類個案之參考。

This case report described a home care nursing experience of caring a victim of cerebral vascular accident who suffered from first time of stroke and just was discharged from admittion. During the period of home nursing care from December 4th, 2009 to June 30th, 2010, we used physical, emotional, family -social, economic, and spiritual aspects of nursing care assessment and instruments. We collected data by the means of physical assessment, communication, observation, individual health education, feedback, and evaluation. After data analysis, we confirmed that the patient had nursing issues including (1) Risk of aspiration; (2) Physical mobility; (3) Preparation for self-care; (4) Caregiver stress overload. In the course of home care, we provided nursing knowledge and available resources in the concept of health promotion and also nursing profession to help patient seizing the golden period of rehabilitation. We aimed at the patient's problems to provide the individual nursing planing and gave the patient confidence and support of improving self-care ability and life quality. We hoped to share this special home care experience of caring this kind of patients.
