運用發展性照護餵食困難早產兒之護理經驗

運用發展性照護於困難餵食之早產兒，使其能儘早由口餵食，陪伴父母、早產兒渡過由鼻胃管餵食至口餵食之時期，以利日後順利生長。本文為運用發展性照護於餵食困難早產兒之照護經驗，對於早產兒吸吮吞嚥不成熟、餵食困難，提供漸進式口腔餵食訓練、營造光線昏暗且安靜的環境、採舒適的臥姿等照護措施，提升個案的舒適感及安全感，有效的改善個案餵食困難及睡眠型態，並鼓勵父母參與照護活動，了解早產兒餵食時出現呼吸急促、疲倦等行為之因應措施，減輕父母親焦慮，增進親子依附關係的連結，提升育兒信心。建議臨床實務在發現早產兒口腔餵食問題時應及早介入，並將家屬納入醫療團隊，在照護個案同時也能顧及家屬的成受與需求，於個案住院期間及早作好出院後的照護準備，使其出院能獲得適切、妥善的持續照護。

Use developmental care in preterm infant of feeding difficulties, as soon as possible so that it can be feeding by mouth, accompanied by parents and premature infant survive by nasal feeding tube feeding to oral feeding of the period, in order to facilitate a smooth growth in the future. ln this paper, nursing experience related to developmental care for difficulty feeding premature infant, for premature infant immature sucking swallowing , feeding difficulties , offers progressive training of oral feeding, creating dimly lit and quiet environment mining and comfortable position and other care measures to enhance the comfort and security cases , cases effectively improve sleep patterns and feeding difficulties ,and to encourage parents to participate in the activities of care, understanding response measures shortness of breath, fatigue and other acts when feeding in premature infant reduce parental anxiety, enhance parent-infant attachment links, enhance parental confidence. Recommended clinical practice found that oral feeding problems in premature infant should be in early intervention and family members covered by medical team, in the case of care but also to take into account the feelings and needs of families, case ready early during hospitalization after discharge care preparations, in case of discharge to obtain appropriate and proper continuum.
