照護一位非哺乳期乳腺炎併膿瘍患者之護理經驗
Nursing Experience of Caring a Patient with Non-puerperal Mastitis related Abscess Formation
摘要
本文描述一位33 歲女性個案，於非哺乳期意外發現右側乳腺炎合併膿瘍，面臨乳房切開引流手術之照護經驗。筆者於2015 年10 月13 日至10月23 日照護期間，運用Gordon 十一項功能性健康型態評估工具進行評估，藉由身體評估、觀察、直接照護及病歷資料查閱，採傾聽及會談方式進行主客觀資料之收集，確立個案主要護理問題有組織完整性受損、急性疼痛、知識缺失及身體心像紊亂等。筆者在經由護理過程深入瞭解個案及其家庭對於疾病及身體外觀改變的壓力與需求，主動協助個案了解非哺乳期乳腺炎併膿瘍之相關知識，引導個案及家屬共同參與傷口照護活動，使其正向面對術後身體外觀的改變，進而恢復住院前的生活模式。期許藉由分享此護理經驗，能提供日後照護此類個案之參考。

This report described the nursing experience care for a 33-year-old woman with non-purperal mastitis related abscess formation underwent breast surgery with incisional drainage. During the nursing care from Oct. 13 to Oct. 23, 2015, Gordon’s eleven functional health patterns was used as the assessment tool. The author collected data through physicial assessment, general observation, directly nursing care, and chart reviews. The identified patient’s problems including impaired tissue integrity, acute pain, lack of knowledge, and body image disturbance. From the course of nursing care, the author understood the stress from the disease and body image changing for the patient and her family. Provided the active assistance for the patient to understand the knowledge of non-purperal mastitis with abscess formation. The author conducted the wound care for the patient and her family and encouraged they to confront the postoperative change of body appearance optimistically. Expected that the patient could recovery life style before hospitalization. The author hopes that this nursing experience care could serve as a reference for other nurses while taking care of similar cases.
