照護一位脊椎手術患者之護理經驗

The Nursing Experience of Caring a Spine Surgery Patient

摘要

本報告探討一位長者因第四及第五脊椎滑脫合併狹窄行減壓及融合手術的護理經驗。護理期間2015年9月13日至2015年9月21日。筆者運用Gordon十一項功能性健康型態評估，以觀察、身體評估、會談，病歷查閱及返家後電訪等方式收集資料，確立個案有焦慮、身體活動功能障礙、情緒性低自尊；依據護理問題擬定個別性護理計畫，教導輔助療法中音樂治療、芳香療法及按摩之協助，以降低其焦慮，並提供關懷性的陪伴，加強家屬的支持度和不間斷的鼓勵、肯定，及教導適當的復健運動，使個案增強自信心和成就感，促使提升身體活動功能並減少負向情緒的產生。
