糖尿病看圖對話衛教工具對於提昇第二型糖尿病人知識、態度、行為、HbA1c及衛教滿意度之成效

摘要


背景:研究顯示增進糖尿病人自我照顧能力,可以有效達到血糖控制且顏色鮮明、圖像的訊息內容較能吸引糖尿病病人之注意,尤其對中老年族群的病人可達到更好的學習效果。而「糖尿病看圖對話」是運用以病人為中心、互動、視覺化的圖示衛教工具,可協助病友發現問題與增強血糖控制的能力。目的:本研究主要探討「傳統口語化」與「糖尿病看圖對話」二種不同衛教模式對病人知識、態度、行為、糖化血色素及衛教滿意度之成效差異。方法:採類實驗設計、立意取樣,於台灣南部某醫學中心新陳代謝科門診共收案128位第二型糖尿病人。實驗組個案接受「糖尿病看圖對話」教學模式,控制組個案接受「傳統口語化」衛教諮詢。於首次收案介入與介入後6個月進行資料收集,分析兩組個案在知識、態度、行為、糖化血色素及衛教滿意度前後測變化之成效差異。結果:「糖尿病看圖對話」衛教工具介入之實驗組個案,介入後6個月在知識、態度、行為及糖化血色素值皆比接受傳統口語化衛教之控制組個案有顯著性的改善(p<0.05),且衛教滿意度方面也顯示實驗組對衛教方式、教材與整體滿意度也顯著較佳(p<0.001)。結論:研究顯示「糖尿病看圖對話」之互動、圖示型的衛教模式,更易讓病人了解與吸收糖尿病相關知識,且提供貼近日常生活的解決方案,可達到第二型糖尿病人的健康促進及獲得較好的血糖控制。

Background: Past studies have found that enhancing self-care ability can effectively achieve blood sugar control and that bright colors and image content can attract the attention of patients with diabetics, especially it can facilitate better learning outcomes for those in middle and older age. The Conversation Map education kit is a patient-centered, interactive and visual iconic health education that can assit patients finding problems and enhancing their blood sugar control. Purposes: The purpose of this study was to examine the distinctive effects of the Traditional verbal education and the Conversation map education kit on knowledge, attitude, practice, glycated hemoglobin and health education satisfaction for type 2 diabetes patients. Methods: This study applied a quasi-experimental design with purposive sampling. A total of 128 patients with type 2 diabetes were admitted to the outpatient metabolism department in a medical center of southern Taiwan. The experimental group received the conversation map education kit while the control group received the traditional verbal education . Data were collected prior to and 6 months after participating in the intervention. Results: Results showed that the experimental group had significantly better scores on knowledge, attitude, behavior, and HbA1C level compared with the control group (p < 0.05). In addition, the satisfaction with health education also showed that the experimental group was significantly better with health education methods, teaching materials and overall satisfaction (p<0.001). Conclusions: This study showed the conversation map education kit is user-friendly for patients to understand and absorb diabetes-related knowledge, and provide solutions that are close to daily life, which can achieve the health promotion better blood sugar control.
