摘要


本專案旨在降低骨科病人術前辨識執行不完整率,以避免對病人造成不可逆的嚴重傷害。改善前,本單位骨科病人術前辨識執行不完整率為94%,經分析後發現造成之主要原因為一、手術部位不易作標記;二、缺乏術前辨識的執行標準;三、缺乏審查制度;四、教育宣導不足。經專案小組透過標準的制定、標記劃法的統一、適當記錄工具與標記筆的使用及持續的查核,再輔以完整教育宣導後,不完整率由改善前的94%降至0%,達本專案的目的。此改善主要目的是保障病人手術安全,避免意外傷害。另外,經由水平推展至本院全體外科後,更提升了本院接受手術之所有病人的安全,是本專案最大的收穫。

Incorrect preoperative identification of orthopedic patients may result in severe, irreversible harm. The aim of this study was to evaluate a system to reduce the number of patients with incomplete preoperative evaluation. Before the implementation, identification procedures were incomplete for 94% of patients. The main problems were: (1) surgical site not clearly marked, (2) no means for enforcing proper preoperative identification standards, (3) no system to check for proper identification, and (4) lack of adequate staff education in the proper procedures. We established clear standards, including a unified method for marking the surgical site, proper use and maintenance of recording instruments and markers, and promoted staff education. After implementation of the project, no identification errors occurred, achieving the aim of the project. It is hoped that this system will improve patient safety.
