一位白血病合併失語症病人的護理經驗
Nursing Experience in Caring for a Patient with Chronic Myeloid Leukemia Conflate Aphasia
摘要
本篇報告是在探討照護一位慢性骨髓性白血病合併失語症病人的護理經驗，護理期間2007年07月02日至2007年08月17日筆者運用會談、傾聽及觀察等技巧以Gordon十一項功能性健康型態進行整體性的評估，確立護理問題包括保護能力改變、言辭溝通障礙及無力感。筆者依據個案之護理問題，教導個案及家屬預防出血措施，善用溝通輔助工具及澄清技巧，確認病人需求增進病人的自我控制感，與語言復健師共同協助語言復健，發揮個別性護理指導和情緒支持的角色功能，同時成功扮演家屬和醫療團隊的溝通橋樑，使個案溝通能力逐漸恢復而出現積極正向行為，協助渡過無力感的危機，重建個案希望、增進生活上品質，期盼此臨床經驗分享，提供護理人員日後照護此類病人的參考。

A 40-year-old man with chronic myeloid leukemia and aphasia was cared for from July 2 to August 17, 2007. Assessment with Gordon's 11 item functional health scale indicated that the patient's major problems included the risk of bleeding secondary to a coagulopathy, impairment of verbal communication, and a sense of hopelessness. Nursing intervention included teaching the patient and family how to prevent bleeding. Effective communication skills and language rehabilitation training were incorporated into daily care. The patient's communication ability improved, and he began to act more positively and appeared more hopeful. Interventions targeted at the patient's specific problems were considered successful.
