A Mother's Grief over Her Son's Out-of-Hospital Cardiopulmonary Arrest

摘要


到院前呼吸心跳停止所造成的突發不可預期死亡,其存活率相當低。本篇旨在敘述照顧一位母親因面臨兒子到院前呼吸心跳停止時,產生的哀傷反應。筆者應用哀傷評估分析、非結構性會談、觀察語言及非語言行為過程收集資料。經評估後護理問題包括心輸出量減少(個案)及預期性哀傷(案母),依Kubler-Ross(1975)之哀傷的五個過程:否認、憤怒、磋商、抑鬱及接受期,描述案母的情緒反應。護理人員除了提供適當的照護,運用哀傷過程護理及運用支持系統,協助個案接受喪子的事實。期藉此護理經驗,讓急診室之護理人員,在面對病患急救過程中而產生失落哀傷的家屬,能得到適當的關注,並獲得完整的支持及照顧,協助順利的渡過哀傷低潮,重新適應新的生活。

Patients who suffer out-of-hospital cardiac arrest have a low chance of survival. This case study describes the nursing experience of grief counseling for a mother whose son was brought to the emergency room after an out-of-hospital cardiopulmonary arrest. Data were collected by observation, interviewing, and nursing assessment. While attempts to resuscitate the patient were ongoing, nursing care for the mother involved addressing her anticipatory grief. The mother was observed for the grief responses described by Kubler-Ross, including denial, anger, bargaining, depression, and acceptance. The author encouraged the mother to talk about her sorrow and guilt feelings. The nursing process involved empathy in helping this woman to face her son's death. This case study is a reminder that, in the emergency room, it is not just the acutely ill or dying patient who requires nursing care. Nurses must also assist family members to address the shock and grief they naturally experience.
