協助一位門診糖尿病人之護理指導經驗

Assisting a Diabetic Outpatient

摘要


本篇個案報告主要是經由護理指導,協助一位糖尿病個案飲食及體重自我控制的護理經驗,護理期間為2008年4月21日到2008年6月30日,經過四次門診護理指導及四次電話訪談,運用Gordon十一項功能性健康型態評估為指引,發現護理問題有:營養不均衡:多於身體需要、焦慮,與健康狀態改變有關、靜態生活型態,與缺乏身體運動對健康有益之知識有關。筆者運用同理心技巧,與個案建立治療性人際關係,針對問題訂定適當目標,增強個案自我照顧能力及信心,進而有效改變飲食和運動行為,達到體重及血糖控制的效果。由此護理過程發現,病人罹患慢性病初期,常會出現焦慮情緒,護理人員及時介入提供疾病控制相關訊息,可增加病人醫療建議遵從性,並主動參與疾病管理。

This case report describes the experience of assisting a patient with self-management of diet and weight control in the Diabetes Outpatient Clinic. From April 21 to June 30, 2008 a man with newly diagnosed diabetes was seen during four outpatient visits and contacted by phone for four interviews. Based on Gordon's 11- item functional health pattern assessment, the patient's problems were identified as poorly balanced nutrition, including too high a caloric intake; anxiety related to changes in health status; sedentary lifestyle; and lack of knowledge of the benefits of exercise. By establishing rapport with the patient and setting appropriate goals for each problem, we improved the patient's confidence and his ability to manage his diabetes and to improve his diet and exercise habits. In the end, he achieved both the weight loss goal and serum glucose control. In the early phases of a chronic illness, it is understandable that patients are anxious about their ability to cope with the disease and its effects on their life style. Timely intervention by the nursing staff, including provision of information on control of the disease, assists patients to participate actively in disease management, which often results in improved compliance.
