縮短急診病人等候手術時間之改善專案

Decreasing Preoperative Delay for Patients in the Emergency Department

摘要


如何讓急診室需手術的病人得到最佳的醫療服務,等候時間是一個重要的關鍵因素。本專案之目的即在縮短急診病人等候手術時間,以提供急診病人最佳的醫療服務效率。本專案經現況分析發現,每位急診病人等候手術時間平均長達40.08分,其原因為:該科沒有可使用的手術房間、醫師無法進行手術、麻醉科人力不足、術前準備未完成、等候檢驗報告、病人情況不穩等。透過手術室、急診及麻醉科醫護人員共同合作下,藉由彈性運用手術房、與麻醉科協調人力配置及重新修訂急診病人送手術室流程與標準等方案之改善後,其等候時間由40.08分縮短至20.6分,達目標值(低於30分鐘)。本專案之改善不僅顧及急診病人之生命安全,提昇急診醫療服務之效率,同時也值得臨床繼續推廣。

A critical factor in the successful management of patients in the emergency department is minimizing the delay in moving those who need surgery to the operating room. We designed a project to reduce this delay. At baseline, the average waiting time was 40.08 minutes. The commonest reasons for delay included 1) no available operating room, 2) surgeons unavailable because they were in surgery, 3) incomplete preoperative preparation, 4) waiting for laboratory data, 5) patients' condition unstable, and 6) surgeons unavailable because of other responsibilities. Staff from the operating rooms, emergency department, and anesthesiology department worked together to implement the project, which includes flexible allocation of operating rooms and revision of standard procedures for sending a patient from the emergency department to the operating room. With these changes, the average waiting time decreased to 20.6 minutes, demonstrating the utility of this project in improving the quality of patient management in the emergency room.
