運用靈性照護模式於一位食道癌末期個案之護理經驗

摘要

死亡如同平日呼吸般的自然，但當面臨死亡時容易激發出個人對靈性的需求。本篇個案報告在探討一位成人食道癌末期病人面臨死亡威脅的靈性照護經驗，筆者於照護期間2010年12月3日～2011年1月7日，以病歷查閱、觀察、會談、直接護理方式及運用Chao、Chen和Yen（2002）靈性護理照護模式，針對與自己共融、與他人共融、與大自然共融、與至高者共融等四面向，進行資料收集，確認個案因面臨死亡的威脅出現靈性困擾之護理問題，應用主動關懷及同理心的人道精神，採用開放式問句的溝通技巧，在醫療團隊的合作下，引導個案表達內心感受、尋求自我生命的意義及價值並給予正向支持，使個案坦然面對死亡、做好臨終準備，獲得靈性上的安適。期望透過此報告的分享，加強臨床實務經驗，並建議舉辦靈性照護及心理輔導技巧相關教育課程，以提昇護理人員對臨終病人靈性照護的評估技巧，提供適切的靈性照護，讓病人有品質的善終。

Death as weekdays breathe like natural, but when faced with death is likely to arouse the personal spiritual needs. This case report is to explore the spiritual care experience, December 3, 2010 to January 7, 2011, during care, access to medical records, observation of the talks, the direct care of the way an adult esophageal terminally ill patients facing death threats and to use Chao, Chen and Yen(2002) spirituality nursing care model for own communion and communion with others, communion with nature, communion with the Most High four-oriented, data collection, and confirmed cases dying threat spiritual distress of care, the application active care and humane empathy, using the communication skills of the open-ended questions, cooperation of the medical team, the patient to express feelings, to seek the meaning and value of the self-life and give positive support to the patient face death, good dying ready, get comfortable on the spiritual. Expectations strengthen clinical practice experience sharing through this report, and suggested organizing spiritual care and counseling skills relevant educational programs to enhance the skills of the nurses' assessment of the spiritual care of dying patients, and to provide appropriate spiritual care, hospice patient quality.
