運用『生命回顧』協助一位肝硬化末期社區老人面對死亡之護理經驗

摘要

老年生活會面對的問題不僅是生理的退化與衰老，亦包含心理調適及社會變遷的衝擊。對老年人而言，生命的意義與價值受到許多挑戰；例如面對及接受死亡的到來，唯有探索生命的意義適應角色與權力的轉變等過程，可以給予適時的支持。『生命回顧』為一可幫助老年人於生命晚期的自我統整，使其更有意義的過完人生的最後旅程。本文探討一位肝硬化末期老人面對死亡之護理照護經驗，照護期間為2011年3月25日至6月10日，藉由整體性評估，引導個案回顧過去生命的意義，及自我內心生命的經歷，與現在的身體及現實做呼應，可協助老人肯定自己、接納自己，使生命達到圓滿的境界。面對高齡化社會的來臨，希望藉此個案照護經驗提供護理人員在老人照護上能有不同之思維，以提昇整體老人照護品質。

Problems faced by aged people include not only physical degeneration and aging but also mental transition and adjustment, as well as shock brought by social and role changes. For aged people, the meaning of their life is challenged by every aspect of life. ”Life Review” is a feasible method to help aged people with self-regulation and makes their life at the end more meaningful. The present article describes a single experience in nursing an aged patient with end-stage cirrhosis from March 25 to June 10, 2011. By holistic assessment and ”life-reviewing”, the nursing processes have focused on helping this patient in coping spiritual distress as well as exploring the meaning of the past life and life experience of the inner self for working in concert with the current body and reality. Finally, assisting in accepting hisself to reach a satisfactory state at the end stage of life has been performed. This single nursing experience along with ”life review” can hopefully provide different thoughts in improving quality of life in aged population with end-stage diseases.
