運用舒適理論於一位口腔癌末期病人之護理經驗

Nursing Experience of Applying Comfort Theory for Caring a Patient with Terminal Oral Cancer

摘要


癌症末期病患,面臨著因癌症所帶來的身、心、靈問題,易出現情緒困擾、無望感、憂鬱,甚至於會有自殺行為。本文描述一位口腔癌末期病人,長期面對腫瘤壓迫引發自殘行為之護理經驗,護理期間為2011年8月26日至9月3日,應用Kolcaba舒適護理理論為基礎,藉由身體評估、訪談、觀察家庭互動、病患出院後仍電話追蹤收集資料,發現病人之主要護理問題有:呼吸困難、疼痛及自我傷害等健康問題,於護理過程中提供姿位引流,傾聽及轉移注意力,教導正向思考及提供宗教支持。期許能透過本文能提供臨床實務參考,進而提升對口腔癌末期病人之照 護品質。

Terminal cancer patients encountered physiological, psychological and spiritualdisturbances and these problems may cause emotional distress, hopelessness,depression,and suicide. This paper described the nursing experiences of an oral cancer patientsuffered from tumor compression induced self-destruction during terminal stage. Thenursing period was from August 26, 2011 to September 3, 2011 and the authors collectedphysical examination, interviewed, observed family interaction, and tracked by telephoneafter hospital discharge along with use of Kolcaba comfort theory. Results from thisnursing experiences found that patients had dyspnea, pain, and self-destruction healthproblems. During the period of hospitalization, the author provided postural drainage,listening, distracting attention, positive thinking, and religious support. This experiencecould be a reference for clinical care to similar patients in the future and improve qualityof care for terminal oral cancer
