提升護理人員組裝高頻震盪呼吸器管路即時率之改善方案

摘要


高頻震盪呼吸器,對急性呼吸窘迫症候群有較佳的治療效果,管路組裝錯誤或延遲,可能使病童喪命。統計2008年8月至2010年7月,管路組裝即時率僅8.7%,經調查發現,主因:護理人員未接受過HFOV管路組裝與使用認知的在職教育及學習教材、管路組裝經驗少、缺乏管路組裝及零件標示,且管路、零件無固定放置位置、缺乏管路組裝操作流程手冊、缺乏相關審查機制,經由訂定管路組裝操作標準步驟、增設管路擺放位置及管路標示方法、規劃在職教育、製作HFOV學理認知手冊、管路組裝操作手冊及教學光碟、制定管路組裝審查機制,管路組裝即時率提升至100%,除增進工作流暢性,提升護理人員專業成就感外,更使病童獲得即時的呼吸照護。

High-frequency oscillatory ventilator is a better treatment of acute respiratory distress syndrome. Piping assembly errors or delays may make children killed. The statistics from August 2008 to July 2010 revealed the piping assembly instant rate in only 8.7%. The survey found that the main reasons included: lacking the on-the-job education and learning materials of HFOV piping assembly and cognitive concept of using, insufficient piping assembly experience, lack of pipe road assembly and parts marking, no fixed pipeline and part placement, lack of pipe assembly operation processes manual, lack of relevant review mechanisms. The improvement includes: setting the standard steps of piping assembly operation, adding the indication of position and marking of pipeline, planning in-service education, producing HFOV theoretical cognitive manual and pipe assembly operations manual and CD, developing piping assembly review mechanism. The piping assembly instant rate is up to 100%. In addition to promoting the working fluency and enhancing nurses' professional accomplishment, children got more immediate respiratory care.
