協助一位血液透析患者渡過喪偶悲傷過程之護理經驗
A Nursing Care Experience of Assisting a Patient with Hemodialysis in Overcoming the Grief of the Loss of Her Husband
摘要
本文是探討一位66歲血液透析患者失去摯愛之護理經驗，護理期間自2012年6月7日至2012年9月28日，經由身體評估、觀察及會談技巧收集資料，發現個案因難以接受伴侶離世而沉溺在懷念逝者情境中導致睡眠障礙、社會退縮、食慾不佳等行為出現，以Gordon十一項健康功能評估進行整體性評估及分析，確立個案有：哀傷功能失常、睡眠型態紊亂、無效性健康維護能力、娛樂活動缺失等健康問題。筆者在護理過程中陪伴個案經歷悲傷過程，運用傾聽及同理心，主動關懷個案需求，透過護理活動的介入，改善個案因喪偶造成睡眠不足之健康問題，並針對飲食習慣改變，給予衛教圖片張貼、藉由飲食記錄表來降低攝取過多高磷食物之行為，且提供休閒活動及鼓勵自我獨立，讓個案重新適應丈夫不在的新環境及將情感重新投注在生活上，此外經由持續性與整體性的關懷照護讓個案成功地重新找回喪偶後的新生活。

This article is to discuss the nursing care experience regarding a 66-year-old female patient who lost her spouse while undergoing hemodialysis treatment. The period of care was from 7th June 2012 to 28th September 2012, and the information was collected through physical assessment, observation, and intensive interviews with patient. The nursing care have found that the patient had the conditions of sleep disorders, social withdrawal and poor appetite because it was unacceptable for her to cope with the fact of her spouse's death. Gordon's 11 Health Functions Assessment was carried out and it came to the conclusion that the patient had health problems of dysfunctional in grieving, disturbed sleep pattern, ineffective health maintenance and deficient diversional activity etc. During the nursing process, I accompanied the patient through the grief moments by listening and provision of empathy; actively cared for the patient, and used the intervention of nursing activities to improve the lack of sleep. Besides, I also arranged to change the patient's eating habits by posting health education pictures, reduced the excessive absorbing of high phosphorus food by diet record sheet and provided the leisure activities to encourage the patient to be self-independent in order to ensure the patient cope with the absence of her husband and place focus on daily life activities. In addition, the patient rediscovered her new widowed life after continuous and holistic nursing care.
