摘要

近年來，伴隨我國人口老化、慢性病盛行，照顧需求大幅增加。因應健康照護體系的變化與醫療、家庭結構的變遷等多重因素的影響，護理專業朝向多元發展，並逐漸擴大執業領域以期滿足老年與長期照護個案的需求，護理人員的長期照護能力逐漸受到重視。台灣目前尚未針對護理人員從事長期照護服務訂定所需具備的核心能力，故本文目的以Walker和Avant所提出的概念分析步驟來進行長期照護領域所需護理能力的概念分析，廣泛性地查證及分析此概念之定義性特徵包括：具備長期照護的相關知識與照護技能、能實際參與長期照護的臨床實務、具備長期照護的管理能力及發展長期照護角色。同時確認產生長期照護之護理能力的前因與後果，從典範案例中了解護理能力的定義性特徵，從邊緣與相反案例中了解長期照護能力概念與其他相關概念的不同。最後描述文獻所呈現的相關實證資料，期透過此概念分析的結果，增進長期照護護理人員對護理能力概念有進一步的認識，並運用於長期照護領域上，進而擴展護理人員的就業職場。

With the rapid growth of aging population and prevalence of chronic conditions, needs for providing formal care have been recognized. Corresponding to changes in social demographic trends and health care advancements, nursing profession has evolved to advance practice and responding to the needs of disabled and aged population. Nursing competence in long term care therefore demands to be specified. The purpose of this paper was to explore the concept of nursing core competences in long term care by using concept analysis approach recommended by Walker & Avant. The authors intended to clarify the defining attributes of nursing competences in long term care: possessing the relevant caring knowledge and skills the clinical practice be participated, the ability in management, and the development of the role identity in the long term care. At the same time, the authors wanted to identify the antecedents and possible consequences of nursing core competences in long term care. A model case is used to demonstrate how nursing core competences in long term care are tied to these critical attributes. Borderline and contradictory cases are presented to differentiate the concepts. This analysis will help nursing staff improve their understanding of the concept of long term care nursing competences and help them apply this knowledge for future clinical practice.
