照顧一位血液透析病人不遵從醫療行為之護理經驗
A Nursing Experience of Taking Care of a Hemodialysis Case who Noncompliance the Medical Treatment
摘要
本文旨在描述照護一位血液透析個案由其它透析中心轉入本院，因接受較嚴謹的透析品質要求時，所產生不遵從醫療行為之護理經驗。護理期間為2013年9月2日至2013年11月6日，筆者藉由觀察、會談、傾聽、檢閱病歷等方式收集資料，並以Gordon十一項健康功能評估進行整體性評估及分析，確立護理問題為：不遵從、體液容積過量、無效性健康維護能力。照護過程中，藉由筆者多次主動關懷，與個案建立良好的醫護夥伴關係，先接受個案的不遵從行為，再漸進式了解其背後的原因，並慢慢提供血液透析相關知識及護理指導，使其能改變對疾病的認知，並協助個案確認問題，且一同討論找出解決問題的共識點；在良好夥伴關係的維持下，個案護理問題能持續性獲得幫助與解決，也讓個案接受現行的透析治療模式，並逐漸適應日後長期血液透析生活，進而提昇整體之生活品質。

This article is for the purpose of recording a hemodialysis case that is transferred to our hospital from other dialysis center, when the case receives the rigorous dialysis quality request which results in a Noncompliance behavior of medical service. The nursing period is from September 2nd to November 6th 2013, the author collects the data by means of observation, discussion, listening and reviewing medical records, and carries out the integrity appraisal and analysis by the Gordon Eleven Functional Health Assessment, the case health problem is confirmed as followings: Noncompliance, excessive body fluid volume, ineffective health maintenance. During the nursing process, the author shows active initiative concern , establishes good nursing partnership with the case, accepts the case's Noncompliance behavior first, then advances to understand the problems behind, and gradually provides the correct hemodialysis knowledge, and enable to change the disease cognition of the case, and assists the case to confirm the problem, then to solve the problem; Under maintenance of the good nursing partnership, the case's nursing problem can be helped and solved, moreover, the case is able to accept the present hemodialysis treatment pattern, and will gradually adapt the longterm hemodialysis in the future, and promote the overall life quality.
