提升手術室護理師術前訪視護理指導完整率～以住院病人為例
An Increase in the Completion Rate of Implementing Nursing Guidance for In-patients in the Preoperative Room
摘要
由於病人對手術訊息不瞭解，對手術因應能力下降。因此手術室護理師至病人單位提供適切完整的術前護理指導，給予正確訊息實為重要，因此激發此專案改善動機。本專案主要目的在提升住院病人術前護理指導完整率、其次為認知正確率、滿意度，及護理師對指導內容認知得分。於2014年7～12月實施。經調查、分析及文獻查證後，選定八項改善方案：統一標準規範、持續品質監測、製作書面輔助工具及多媒體影片、制定在職教育辦法、舉辦示範教學、定期經驗分享、增訂初次術訪人員訓練計畫。主要結果發現住院病人術前訪視護理指導完整率由67.3%提升至98.4%，而住院病人術前護理指導認知正確率由62.7%提升至97.5%、滿意度由77.3%提升至95.5%、護理師對護理指導內容的認知得分由69.8分提升至100分；故本專案除提升護理指導完整率外，亦提升住院病人術前護理指導認知正確率及護理師的認知得分，進而提升護理之專業認知，同時病人的滿意度亦獲得極大之改善，對護理品質具正面之肯定。

When patients do not understand their surgery, they may cope with their surgery with limitations. Therefore, the operating room nurse should provide appropriate preoperative care and guidance information for the patients is indeed important, which is the underlying motive for this project. The main purpose of this project was to improve the rate of implementing nursing guidance by operating room nurses for the inpatients prior to their surgery. The project has other goals which include an attempt to completed rate for preoperative visits and improved rates in knowledge accuracy, satisfaction, and the patient's understanding of nursing guidance with accurate content scor es. The project was carried out from July to December in 2014. After using survey investigation and analysis with current literature for verification, the eight improvement programs were selected: a unified care standards, continuous quality monitoring, the creation of written aids and multimedia videos, in-service education and training, ways of teaching in skill demonstration, regularly experience sharing, personnel training and updating the initial preoperative visit plan. The main results showed that the completion rate of nursing guidance for in-patients preoperative visit has increased from 67.3% to 98.4%. The accuracy in the knowledge score after the project has improved from 62.7% to 97.5%. The satisfaction rate has increased from 77.3% to 95.5%. Nurse nursing instruction contents was scored from 69.8 points up to 100 points. Therefore, this project was able to increase the completion rate of the nursing guidance, but also to improve both the patient's and nurse's knowledge accuracy, and thus enhance the professional care by improve patient's knowledge and satisfaction, so as to achieve great improvement in the quality of care with positive affirmation from the patients.
